Наличие коморбидной ФМ существенно влияет на течение ревматоидного артрита (РА).Цель исследования – оценка влияния ФМ на показатели активности РА, особенности болевого синдрома и качество жизни (КЖ) пациентов.Пациенты и методы. В исследование включены пациенты с подтвержденным диагнозом РА. Активность РА оценивалась по индексу DAS28. ФМ диагностировалась согласно критериям ACR 2016 г. с применением опросника диагностики ФМ (FSQ). Пациенты также заполняли опросники центральной сенситизации – ЦС (CSI) и PainDETECT (PD). Оценка интенсивности боли в покое проводилась по визуальной аналоговой шкале (ВАШ, 10 см), оценка КЖ – с помощью опросников FIQR и EQ-5D.Результаты и обсуждение. Обследовано 55 пациентов с РА. ФМ диагностирована у 24 из них (43,6%). Активность РА при наличии и отсутствии ФМ (ФМ+ и ФМ- соответственно) была высокой: индекс DAS28 в группе ФМ+ составил в среднем 4,58 балла, а в группе ФМ- он достигал 5,14 балла (p=0,227). Медиана интенсивности боли по ВАШ в группах ФМ+ и ФМ- равнялась 6,5 и 4,0 см соответственно (p=0,017). Симптомы ЦС у пациентов с ФМ встречались чаще, чем у пациентов без ФМ (p=0,004). Медиана оценки по опроснику PD в группах ФМ+ и ФМ- достигала 17,5 и 11,0 баллов соответственно (p=0,015). У пациентов с ФМ были выявлены более низкие показатели КЖ как по EQ-5D (p=0,037), так и по FIQR (p&lt;0,001).Заключение. ФМ является серьезным бременем для пациентов с РА. Болевой синдром при наличии ФМ имеет большую интенсивность и распространенность, сопровождается невропатическими дескрипторами, что значительно ухудшает КЖ у пациентов с РА.</p></abstract><trans-abstract xml:lang="en"><p>The prevalence of fibromyalgia (FM) in the general population is 2–8%, and among patients with rheumatic diseases it is 2–3 times higher. The presence of comorbid FM significantly affects the course of rheumatoid arthritis (RA).Objective: to assess the effect of FM on RA activity indicators, features of pain syndrome, and quality of life (QoL) of patients.Patients and methods. The study included patients with a confirmed diagnosis of RA. RA activity was assessed by the DAS28 index. FM was diagnosed according to the 2016 ACR criteria using the FM Diagnostic Questionnaire (FSQ). Patients also completed the central sensitization questionnaires – CS (CSI) and PainDETECT (PD). Pain intensity at rest was assessed using a visual analog scale (VAS, 10 cm), QoL was assessed using the FIQR and EQ-5D questionnaires.Results and discussion. 55 patients with RA were examined. FM was diagnosed in 24 of them (43.6%). The median score on the PD questionnaire in the FM+ and FM- groups reached 17.5 and 11.0 points, respectively (p=0.015). Patients with FM had lower QoL scores both in EQ-5D (p=0.037) and FIQR (p&lt;0.001).Conclusion. FM is a serious burden for RA patients.
