Применение анальгетиков имеет принципиальное значение для ведения больных остеоартритом (ОА), поскольку при этом заболевании до настоящего времени не разработана единая система патогенетической терапии. При иммуновоспалительных ревматических заболеваниях (ИВРЗ), таких как ревматоидный артрит, эффективная патогенетическая терапия позволяет успешно контролировать воспалительную активность. Однако во многих случаях у пациентов с ИВРЗ неприятные симптомы (боль, утомляемость, плохое общее самочувствие и др.) сохраняются даже на фоне ремиссии и низкой воспалительной активности. Это связано со стойкими структурными изменениями, вторичным ОА, центральной сенситизацией и сопутствующей фибромиалгией. Поэтому примерно 50% больных с ИВРЗ, получающих современную патогенетическую терапию, требуется дополнительное применение анальгетиков. Терапия скелетно-мышечной боли при РЗ должна носить комплексный характер, включать медикаментозные средства с разным механизмом действия и немедикаментозные методы. Первой линией терапии являются нестероидные противовоспалительные препараты (НПВП), обладающие хорошим анальгетическим и противовоспалительным потенциалом. Выбор конкретного НПВП должен базироваться на индивидуальной оценке клинической ситуации и наличии факторов риска возможных осложнений. Среди НПВП следует выделить ацеклофенак – эффективный препарат с благоприятным профилем безопасности, что подтверждено большим числом клинических исследований и реальной клинической практикой.</p></abstract><trans-abstract xml:lang="en"><p>Control of chronic pain is one of the main elements of the complex therapy of rheumatic diseases (RD). The use of analgesics is of fundamental importance for the management of patients with osteoarthritis (OA), since a unified system of pathogenetic therapy has not yet been developed for this disease. In immunoinflammatory rheumatic diseases (IRDs), such as rheumatoid arthritis, effective pathogenetic therapy can successfully control inflammatory activity. However, in many cases, in patients with IRDs, unpleasant symptoms (pain, fatigue, poor general well-being, etc.) persist even during remission and low inflammatory activity. This is associated with persistent structural changes, “secondary” OA, central sensitization, and concomitant fibromyalgia. Therefore, approximately 50% of patients with IRDs receiving modern pathogenetic therapy require additional use of analgesics. Therapy for musculoskeletal pain in RD should be complex, include drugs with different mechanisms of action and non-drug methods. Nonsteroidal anti-inflammatory drugs (NSAIDs) are the first line therapy, they have good analgesic and anti-inflammatory potential. The choice of a specific NSAID should be based on an individual assessment of the clinical situation and the presence of risk factors for possible complications.
