Для лечения РА применяется широкий спектр базисных противовоспалительных препаратов, включая генно-инженерные биологические препараты (ГИБП) и ингибиторы Янус-киназ (иJAK). Однако даже при использовании ГИБП и иJAK цели терапии РА удается добиться лишь у 40–60% пациентов. Недостаточный ответ на ГИБП и иJAK требует переключения пациентов на другие препараты как из одной группы, например ингибиторов фактора некроза опухоли α, так и на препараты с другим механизмом действия. Необходимость изменения терапии может быть связана с ее неэффективностью вследствие различных иммунных, генетических и эпигенетических механизмов, с развитием неблагоприятных реакций, а также с коморбидной патологией. До настоящего времени не определены предикторы, позволяющие однозначно прогнозировать эффективность того или иного класса ГИБП и иJAK, а также оценивать необходимость и стратегию смены терапии.</p><p>В обзоре рассмотрены эффективность различных классов ГИБП и иJAK при РА, частота и факторы риска, ассоциированные с необходимостью переключения пациентов на другие препараты, роль хемокинов как перспективных маркеров ответа на лечение РА.</p></abstract><trans-abstract xml:lang="en"><p>The goal of treatment of rheumatoid arthritis (RA) is to achieve remission or low disease activity. A wide range of disease-modifying antirheumatic drugs is used for the treatment of RA, including biological disease-modifying antirheumatic drugs (bDMARDs) and Janus kinase inhibitors (JAKi). However, even with the use of bDMARDs and JAKi, this goal can be achieved only in 40–60% of patients. Insufficient response to bDMARs and JAKi is the reason for switching to other drugs from the same group, such as tumor necrosis factor-α inhibitors, and to drugs with a different mechanism of action. The need to change therapy may be associated with its ineffectiveness due to various immune, genetic and epigenetic mechanisms, with the development of adverse reactions, as well as with comorbid pathology.
