Основная цель лечения – достижение ремиссии или низкого уровня активности болезни. Среди трудно поддающихся лечению пациентов выделяют три группы: пациенты, лечение которых не соответствует стратегии «Лечение до достижения цели» (Treat-to-Target, Т2Т) в силу либо недостаточной активности врача, либо несоблюдения режима лечения больным; пациенты, у которых не распознаны центральные механизмы боли; пациенты с «истинно рефрактерным РА».</p><p>Цель исследования – оценка внедрения стратегии Т2Т в реальную клиническую практику ревматологов, понимания врачами механизмов развития труднолечимого РА.</p><sec><title>Материал и методы</title><p>Материал и методы. Исследование включало три этапа: 1) оценка времени подсчета врачами индекса DAS-28; 2) анонимный онлайн-опрос 43 ревматологов Екатеринбурга и Свердловской области; 3) оценка понимания пациентами показателя «Общая активность заболевания». Результаты исследования представлены в виде описательной статистики.</p></sec><sec><title>Результаты и обсуждение</title><p>Результаты и обсуждение. Среднее время подсчета индекса DAS28 занимает в среднем 1 мин 16 с, однако правильно вычислили его только 42% ревматологов. Выявлена проблема недостаточного внимания врачей к субъективной составляющей индекса DAS28 и возможной центральной сенситизации как причине боли. Крайне трудной для понимания и неоднозначной является оценка пациентом своего здоровья.</p></sec><sec><title>Заключение</title><p>Заключение. На сегодня реализацию стратегии Т2Т при ведении пациентов с РА нельзя признать оптимальной. Нередко индекс DAS28 используется чисто механически, без учета возможной центральной сенситизации, что может привести к гипердиагностике высокой воспалительной активности РА, а также к неоправданной смене проводимой терапии.</p></sec></abstract><trans-abstract xml:lang="en"><p>Rheumatoid arthritis (RA) is an incurable but potentially controllable disease. The main goal of treatment is to achieve remission or low disease activity. Among difficult-to-treat patients, three groups are distinguished: patients whose treatment does not comply with the principle of “treat to target” (T2T) due to insufficient activity of the doctor, or due to non-compliance with the treatment regimen by the patient; cases where the central mechanisms of pain are not recognized; patients with “truly refractory RA.”</p><sec><title>Objective</title><p>Objective: to evaluate the implementation of the T2T strategy in the real-world clinical practice of rheumatologists, and the physicians' understanding of the mechanisms of the development of a difficult-to-treat RA.</p></sec><sec><title>Material and methods</title><p>Material and methods. The study included three stages: 1) assessment of the duration of counting of DAS-28 index by doctors; 2) anonymous online survey of 43 rheumatologists from Yekaterinburg and the Sverdlovsk region; 3) assessment of patients' comprehension of the indicator “General activity of the disease”. The results of the study are presented in the form of descriptive statistics.</p></sec><sec><title>Results and discussion</title><p>Results and discussion. The average counting time of the DAS28 is 1 min 16 s, but only 42% of rheumatologists calculated it correctly. The problem of physicians' insufficient attention to the subjective component of the DAS28 index and possible central sensitization as a cause of pain was revealed. Assessment of health is difficult and ambiguous for the patients.</p></sec><sec><title>Conclusion</title><p>Conclusion. The implementation of the T2T strategy in the treatment of patients with RA cannot be considered optimal today.
