Проведен апостериорный анализ результатов международного многоцентрового рандомизированного плацебоконтролируемого клинического исследования II фазы BCD-180-2/ELEFTA (ClinicalTrials.gov NCT05445076), включавшего пациентов с активным р-аксСпА, которые ранее не получали лечения генно-инженерными биологическими или таргетными препаратами и являлись носителями человеческого лейкоцитарного антигена – HLA-В27. Ранее опубликованы основные результаты клинического исследования ELFTA, характеризующие клиническую эффективность и безопасность СЕНИ на протяжении 48 нед терапии.</p><p>Сравнительный post hoc анализ эффективности СЕНИ проведен на точке 48 нед в подгруппах пациентов с наличием (n=48) и отсутствием (n=212) ВСП, получавших терапию СЕНИ в дозе 5 или 7 мг/кг либо плацебо/СЕНИ 5 мг/кг. В качестве параметров эффективности использовались динамика индексов ASDAS, BASFI, интенсивности боли в спине и ночной боли в спине по числовой рейтинговой шкале (ЧРШ) и уровня СРБ, а также частота достижения ответов по ASAS40, ASAS5/6, клинически значимого улучшения по ASDAS (ASDAS-CII) и низкой активности или неактивного заболевания по ASDAS.</p><p>Дополнительная оценка переносимости терапии включала анализ частоты ухудшений имеющихся ВСП и появления ВСП de novo.</p></sec><sec><title>Результаты и обсуждение</title><p>Результаты и обсуждение. На фоне применения СЕНИ в дозах 5 и 7 мг/кг в обеих подгруппах пациентов с наличием и отсутствием ВСП аксСпА наблюдались положительная динамика уровня СРБ c уменьшением активности заболевания по индексу ASDAS и функциональных нарушений по BASFI, а также достижение ответа по ASAS40 и ASAS5/6 и уменьшение интенсивности боли в спине и ночной боли в спине по ЧРШ. Статистически значимое превосходство над плацебо продемонстрировано в подгруппе СЕНИ без ВСП по всем параметрам эффективности и в подгруппе СЕНИ с ВСП по ряду оценочных точек уже в первые недели терапии и вплоть до недели 24. В дальнейшем вплоть до недели 48 во всех подгруппах СЕНИ, независимо от наличия ВСП, отмечено сохранение достигнутого клинического эффекта, а в подгруппах плацебо – увеличение эффекта в связи с переключением на терапию активным препаратом на неделе 24. В подгруппе СЕНИ с ВСП наблюдался численно более выраженный клинический эффект в начале терапии без статистически значимых различий с подгруппой СЕНИ без ВСП.</p><p>Во всех подгруппах продемонстрирован благоприятный профиль переносимости терапии СЕНИ. Не выявлено ни одного случая развития ВСП de novo.</p></sec><sec><title>Заключение</title><p>Заключение. СЕНИ характеризуется высокой стабильной клинической эффективностью и хорошей переносимостью на протяжении 48 нед терапии активного р-аксСпА независимо от наличия или отсутствия ВСП. При этом применение СЕНИ сопровождается тенденцией к более быстрому и значимому развитию клинического эффекта в первые недели терапии в подгруппе пациентов с ВСП.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Objective</title><p>Objective: to assess the impact of extra-articular manifestations (EAM) on the achievement of clinical effect of seniprutug (SENI) in patients with active radiographic axial spondyloarthritis (r-axSpA) during 48 weeks of therapy.</p></sec><sec><title>Material and methods</title><p>Material and methods. A post hoc analysis of results from an international multicenter randomized placebo-controlled phase II clinical trial (CT) BCD-180-2/ELEFTA (ClinicalTrials.gov NCT05445076) involving HLA-B27 positive biologic-naive patients with active r-axSpA was conducted. The main results of the CT ELEFTA on the clinical efficacy and safety of SENI over 48 weeks of therapy have been previously published.</p><p>A comparative post hoc analysis of SENI efficacy was conducted at week 48 in subgroups of patients with presence (n=48) and absence (n=212) of EAM, who received SENI at doses of 5 mg/kg or 7 mg/kg or Placebo/SENI 5 mg/kg. Efficacy parameters included dynamics in the ASDAS, BASFI, as well as total back pain intensity and night back pain scores on a numerical rating scale (NRS), and C-reactive protein (CRP) levels. Additionally, the frequency of achieving ASAS40, ASAS5/6 responses, ASDAS clinically important improvement (ASDAS-CII), and low disease activity or inactive disease according to ASDAS was analyzed.</p><p>The assessment of treatment tolerability included analysis of the frequency of worsening of existing EAM and the occurrence of cases of EAM de novo.</p></sec><sec><title>Results and discussion</title><p>Results and discussion. In both subgroups of patients with and without EAM of axSpA we observed decrease of CRP level with improvement of the disease activity according to the ASDAS index and improvement of functional impairments according to the BASFI index, as well as achievement of ASAS40 and ASAS5/6 responses and a reduction in of the total back pain and night back pain according to the NRS. Statistically significant superiority over placebo was demonstrated in the SENI subgroup without EAM at all efficacy parameters and at several assessment points in the SENI subgroup with EAM already in the first weeks of therapy and up to week 24. Further, the achieved clinical effect was maintained, up to week 48, in all SENI subgroups regardless of the presence of EAM, while in placebo subgroups, an increase in effect was observed due to switching to active drug therapy at week 24. In the SENI subgroup with EAM, a numerically more pronounced clinical effect was noted at the beginning of therapy without statistically significant differences compared to the SENI subgroup without EAM.</p><p>In all studied subgroups, a favorable tolerability profile of SENI therapy was demonstrated. No EAM de novo was registered.</p></sec><sec><title>Conclusion</title><p>Conclusion.
