Он развивается в случае недостаточно эффективного контроля острой боли, на фоне персистирующего воспаления, дегенеративных процессов, дисфункции ноцицептивной системы и психоэмоциональных нарушений. Ведущей стратегией анальгетической терапии при РЗ является мультимодальный подход с использованием препаратов и методов лечения, направленных на основные элементы патогенеза боли. Для обезболивания при заболеваниях суставов и позвоночника, как правило, применяются парацетамол, нестероидные противовоспалительные препараты (НПВП) и опиоиды. Одним из путей повышения эффективности противоболевой терапии является рациональная комбинация анальгетиков. В частности, сочетанное применение НПВП и парацетамола повышает результативность лечения на 30–40%. Учитывая, что большинство пациентов с РЗ – люди старших возрастных групп, имеющие коморбидные заболевания сердечно-сосудистой системы, большой интерес вызывает использование в комбинированных препаратах напроксена. Этот НПВП обладает высоким анальгетическим потенциалом и одновременно наилучшим профилем кардиоваскулярной безопасности. Поэтому новый препарат, представляющий собой комбинацию напроксена 275 мг и парацетамола 500 мг, который недавно вошел в российскую медицинскую практику, может рассматриваться как один из наиболее удачных инструментов для контроля скелетно-мышечной боли при РЗ.</p></abstract><trans-abstract xml:lang="en"><p>Chronic pain is the most burdensome manifestation of rheumatic diseases (RD), a syndrome with a complex multifactorial pathogenesis and a serious threat to the patient’s health and life. It develops when acute pain is insufficiently controlled, against the background of persistent inflammation, degenerative processes, dysfunction of the nociceptive system, and psycho-emotional disturbances. The leading strategy of analgesic therapy in RD is a multimodal approach using drugs and treatment methods targeting the main elements of pain pathogenesis. For pain relief in diseases of the joints and spine, paracetamol, nonsteroidal anti-inflammatory drugs (NSAIDs), and opioids are generally used. One way to increase the effectiveness of analgesic therapy is a rational combination of analgesics. In particular, combined use of NSAIDs and paracetamol increases treatment efficacy by 30–40%. Given that most patients with RD are older individuals with comorbid cardiovascular diseases, the use of naproxen in combination products is of great interest. This NSAID has high analgesic potential and, at the same time, the best cardiovascular safety profile.
