Проведен систематический поиск литературы в базах данных PubMed и Embase. Отобрано 11 исследований реальной клинической практики. Анализ данных выполнен в среде R v.4.3.0 с использованием модели метаанализа со случайными эффектами и метарегрессионного анализа. В качестве ковариат выбраны доля пациентов с диффузной формой заболевания и доля пациентов с интерстициальным поражением легких. Для прогнозирования показателей для Российской Федерации приняты значения доли больных 40 и 60% соответственно. Оценка риска систематической ошибки проведена с помощью инструментария ROBINS-Iv2. </p></sec><sec><title>Результаты и обсуждение</title><p>Результаты и обсуждение. По итогам метарегрессионного анализа с бивариантным подходом смертность при ССД в России оценена в 26,1 случая на 1000 пациентов в год (95% доверительный интервал, ДИ 17,3–39,3). Стандартизированное отношение смертности составило 4,2 (95% ДИ 2,6–6,5), что означает превышение риска смерти в 4,2 раза по сравнению с общей популяцией. Полученные значения согласуются с международными данными и подтверждают определяющую роль диффузной формы и поражения легких в формировании смертности. </p></sec><sec><title>Заключение</title><p>Заключение. ССД в России является заболеванием с высоким риском преждевременной смертности. Разработанная метарегрессионная модель позволяет получить валидные предварительные оценки бремени болезни в условиях отсутствия национального регистра. Полученные результаты подчеркивают необходимость ранней диагностики поражения легких, стандартизации скрининга и создания национального регистра для мониторинга течения заболевания и эффективности терапии. </p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Objective</title><p>Objective: to develop a population-based mortality model for patients with systemic sclerosis (SSc) and to assess the disease burden (mortality indicators) for the Russian Federation.</p></sec><sec><title>Material and methods</title><p>Material and methods. A systematic review was conducted in the PubMed and Embase databases. Eleven real-world clinical practice studies were selected. Data analysis was performed in R v.4.3.0 using a random-effects meta-analysis model and meta-regression analysis. Covariates included the proportion of patients with diffuse disease and the proportion of patients with interstitial lung disease. For forecasting indicators for the Russian Federation, the proportions of patients were assumed to be 40% and 60%, respectively. Risk of bias was assessed using the ROBINSIv2 tool.</p></sec><sec><title>Results and discussion</title><p>Results and discussion. Based on bivariate meta-regression analysis, SSc mortality in Russia was estimated at 26.1 deaths per 1000 patientyears (95% confidence interval, CI 17.3–39.3). The standardized mortality ratio was 4.2 (95% CI 2.6–6.5), indicating a 4.2-fold higher risk of death compared with the general population. The obtained estimates are consistent with international data and confirm the determining role of diffuse disease and lung involvement in shaping mortality.</p></sec><sec><title>Conclusion</title><p>Conclusion. SSc in Russia is associated with a high risk of premature mortality. The developed meta-regression model provides valid preliminary estimates of disease burden in the absence of a national registry.
