Основной механизм действия фебуксостата заключается в подавлении активности ксантиноксидазы, что приводит к снижению уровня мочевой кислоты в сыворотке крови. Предполагается, что не менее важные механизмы действия препарата связаны с участием в иммунных процессах и способствуют подавлению как локального, так и системного воспаления, он обладает также антиоксидантным эффектом. Препарат следует считать безопасным у пациентов с сердечно-сосудистыми заболеваниями, и он характеризуется низкой гепатотоксичностью. Помимо установленного ренопротективного эффекта фебуксостата, заключающегося в замедлении прогрессирования хронической болезни почек, в настоящее время рассматриваются возможности его применения при других заболеваниях, в частности при остеоартрите, заболеваниях нервной и легочной систем, воспалительных заболеваниях кишечника и COVID-19. </p></abstract><trans-abstract xml:lang="en"><p>Febuxostat was originally developed as a treatment for hyperuricemia in patients with gout; however, studies in recent years have demonstrated broader therapeutic potential beyond its urate-lowering effect. The main mechanism of action of febuxostat is inhibition of xanthine oxidase activity, which leads to a reduction in serum uric acid levels. It is assumed that other mechanisms of action of not lesser importance are related to involvement in immune processes and contribute to suppression of both local and systemic inflammation; it also has an antioxidant effect. The drug should be considered safe in patients with cardiovascular diseases and is characterized by low hepatotoxicity.
