Представлен клинический случай успешного применения ритуксимаба (РТМ) в комплексной терапии пациентки азиатского происхождения с высокоактивной системной красной волчанкой (СКВ), формированием люпус-нефрита и ретинопатии Пурчера (РП), рефрактерных к иммуносупрессивной терапии. Отмечалось не только снижение клинических, иммунологических показателей активности и протеинурии (частичный почечный ответ), но и значимое улучшение перфузии сетчатки по данным оптической когерентной томографии-ангиографии через 5 мес после инфузии РТМ, что позволило снизить суточную дозу преднизолона. Приведенное наблюдение подтверждает эффективность РТМ при рефрактерной РП, ассоциированной с СКВ.

We present a clinical case of successful use of rituximab (RTM) as part of complex therapy in an Asian female patient with highly active systemic lupus erythematosus (SLE), lupus nephritis, and Purtscher retinopathy (PR) refractory to immunosuppressive therapy. This observation demonstrates not only a reduction in clinical and immunological disease activity parameters and proteinuria, indicating a partial renal response, but also a significant improvement in retinal perfusion according to optical coherence tomography angiography 5 months after RTM infusion, which made it possible to reduce the daily prednisolone dose. This case confirms the efficacy of RTM in refractory SLE-associated PR.
