Обсуждаются трудности проведения адекватной и безопасной терапии у таких пациентов. Представлены основные положения клинических рекомендаций по рациональному применению нестероидных противовоспалительных препаратов (НПВП) 2015 г., оценка факторов риска НПВП-ассоциированных осложнений. Обсуждается место медленнодействующих при ОА препаратов (SYSDOA), расхождение взглядов различных национальных и международными сообществ специалистов на их эффективность. Приведены последние рекомендации по ведению больных ОА международного комитета ESCEO 2014 г., в которых подтверждается необходимость назначения SYSАDOA, причем в качестве начального этапа лечения. Приводятся данные о свойствах и функции синовиальной жидкости в норме и при ОА. На основании данных клинических и экспериментальных исследований показано, что, помимо улучшения лубрикационных свойств синовиальной жидкости, экзогенный гиалуронан обладает многими антикатаболическими и анаболическими свойствами, влияет на патогенетически значимые факторы при ОА. Приводятся результаты рандомизированных клинических исследований (РКИ), в которых сравнивали гиалуронаны с различной молекулярной массой. Представлены данные РКИ о клинической эффективности гилана G-F 20 и достоверном уменьшении боли у пациентов с ОА при его использовании. Продемонстированы преимущества в выживаемости обезболивающего эффекта гилана G-F 20 по сравнению с внутрисуставным введение глюкокортикоидов. Приведены имеющиеся данные о структурно-модифицирущем воздействии гилана G-F 20.</p></abstract><trans-abstract xml:lang="en"><p>The paper gives data on the prevalence of osteoarthritis (OA) and the rate of comorbidities in this disease. It discusses problems of adequate and safe therapy in these patients. The basic provisions of the 2015 clinical guidelines for the rational use of nonsteroidal antiinflammatory drugs (NSAIDs), as well as assessment of risk factors for NSAID-related complications are presented. The place of symptomatic slow-acting drugs for OA (SYSАDOA) and a clash of opinions as to their efficacy among different national or international communities of specialists are discussed. The 2014 ESCEO International Committee guidelines for the management of OA patients, which confirm the necessity of prescribing SYSDOA as initial treatment, are presented. The properties and function of synovial fluid in health and OA are depicted. Clinical and experimental findings have shown that, by improving the lubricating properties of synovial fluid, exogenous hyaluronon has many anticatabolic and anabolic effects and affects pathogenetically significant factors in OA. The results of randomized clinical trials (RCTs) comparing hyaluronans of different molecular weight are given. There are data of RCTs on the clinical efficacy and structuremodifying activity of hylan G-F 20 and on its significant pain relief in OA patients.
