Место фебуксостата в терапии подагры

Аннотация

Курация пациентов с подагрой предполагает обязательное назначение патогенетической терапии – уратснижающих препаратов, среди которых наиболее широко применяются ингибиторы ксантиноксидазы: аллопуринол и фебуксостат. Согласно различным национальным и международным рекомендациям, аллопуринол является препаратом первой линии лечения подагры. Однако существует большая когорта пациентов, у которых применение фебуксостата не только оправданно, но и предпочтительно. В первую очередь это пациенты с непереносимостью аллопуринола и высоким риском развития тяжелых кожных реакций. Высокий риск летальности и низкая вероятность достижения целевого уровня мочевой кислоты при назначении аллопуринола наблюдаются у больных со сниженной функцией почек. Учитывая, что фебуксостат обладает выраженным нефропротективным действием назначение его таким больным будет более эффективным способом достижения нормоурикемии. В то же время наличие патологии сердечно-сосудистой системы, хотя и требует дополнительной осторожности при выборе терапии, не должно стать причиной отказа от применения фебуксостата, поскольку результаты многих работ не только не подтвердили увеличение риска сердечно-сосудистых осложнений на фоне терапии этим препаратом, но и показали, что он обладает кардиопротективными свойствами.
