Резолюция консенсуса экспертов Российской Федерации по диагностике и лечению остеоартрита 2022

Остеоартрит (ОА) — наиболее распространенное заболевание суставов, что обусловлено увеличением продолжительности жизни населения планеты. Каждый 2-й пациент старше 50 лет страдает ОА коленных или тазобедренных суставов, и это напрямую связано с увеличением нагрузки на здравоохранение. Накопленные данные о коморбидном профиле больных ОА диктуют необходимость выделения фенотипов заболевания для оказания персонифицированной помощи. Дифференциального подхода требуют и отдельные клинические проявления ОА: лечение болевого синдрома должно проводиться с учетом психоэмоционального профиля пациента и возможности вовлечения в процесс ноцицептивных путей. Вместе с тем из-за неоднозначности существующих клинических рекомендаций большинство экспертов отмечают ряд сложностей при назначении лечения больным ОА.

В резолюции представлено согласованное мнение экспертов об алгоритмах ведения больных ОА, начиная с первичного звена, с поэтапным подключением смежных специалистов. Рассмотрены некоторые вопросы персонифицированного подхода в зависимости от наличия коморбидной патологии и степени выраженности отдельных симптомов заболевания. Отмечена перспективность комбинированного использования фармакологических и немедикаментозных методов лечения; сделан акцент на важности проведения реабилитационных мероприятий па первом этапе оказания помощи больным ОА, задолго до развития структурных изменений. Предложенные алгоритмы ведения больных можно рассматривать как основу для будущих рекомендаций ведения пациентов с данной патологией.
