Сочетание системной красной волчанки и псориатического артрита : данные литературы и описание случая

Аннотация
Системная красная волчанка (СКВ) – аутоиммунное системное воспалительное заболевание с разнообразными клиническими проявлениями. Имеются описания перекрестных синдромов, когда одновременно регистрируются признаки нескольких системных заболеваний соединительной ткани, таких как ревматоидный артрит, системный склероз, дерматомиозит и синдром Шёгрена. Псориаз относится к хроническим заболеваниям кожи с мультифакторным генезом, который определяется генетическими, иммунологическими факторами и влиянием окружающей среды. Примерно у каждого третьего больного с кожным псориазом развивается воспалительное поражение суставов – псориатический артрит (ПсА). Сочетание СКВ и ПсА встречается редко, описаний подобных клинических ситуаций крайне мало. Особый вариант СКВ (подострая кожная красная волчанка) может имитировать поражение кожи при псориазе, что вызывает диагностические трудности.
В статье приведены данные литературы, касающиеся сочетания СКВ, ПсА и псориаза. Обсуждаются особенности дифференциальной диагностики и терапевтической тактики при комбинации двух нозологических форм. Представлено собственное клиническое наблюдение сочетания СКВ и ПсА, а также несколько описаний подобных случаев, имеющихся в литературе.
