Фармакоэкономический анализ применения биологических препарато в в лечении ревматоидного а ртрита

Аннотация

Учитывая наличие нескольких альтернативных генно-инженерных биологических препаратов (ГИБП) для лечения ревматоидного артрита (РА), стоимость годового курса терапии которыми превышает затраты на высокотехнологичные хирургические вмешательства, проведено фармакоэкономическое исследование, обосновывающее оптимальный выбор конкретного препарата. Как показал фармакоэкономический анализ, применение ГИБП этанерцепт (ЭТЦ) позволяет уменьшить стоимость годового курса лечения каждого больного РА в среднем на 84 764–481 622 руб., значительно увеличить число пациентов, обеспеченных современной терапией без выделения дополнительных ресурсов. Перевод 100 больных РА на схему лечения, включающую ЭТЦ, дает возможность дополнительно пролечить 14–78% пациентов с РА в рамках того же бюджета. Представленные результаты с учетом данных об отсутствии значимых различий в эффективности и безопасности ГИБП, применяемых для лечения РА (Национальные рекомендации по лечению РА, рекомендации EULAR, ряд метаанализов рандомизированных клинических контролируемых исследований), позволяют заключить, что схема лечения, включающая ЭТЦ, является наиболее предпочтительной. Аналогичные результаты получены и при анализе эффективности затрат, основанием для которого послужил единичный метаанализ G.J. Bergman и соавт., продемонстрировавший различия в эффективности ГИБП. В данном случае применение ЭТЦ также являлось наиболее предпочтительным, поскольку позволяет минимизировать издержки системы здравоохранения на достижение ответа по критериям ACR20 и ACR50. Это преимущество ЭТЦ приведет к максимальному сокращению числа госпитализаций, социальных выплат и иных расходов, связанных с ведением больных РА.
