Комплексное применение противовоспалительных препарaтов и миорелаксантов в лечении пациентов с анкилозирующим спондилитом в амбулаторной практике

Цель исследования – оценка разных схем лечения анкилозирующего спондилита (АС). Выполнен ретроспективный анализ особенно- стей применения местных и пероральных нестероидных противовоспалительных препаратов (НПВП), миорелаксантов в амбулаторной практике.

Материал и методы. В исследование включено 96 пациентов с АС, госпитализированных в отделение ревматологии ГУЗ «Областная клиническая больница» (Саратов) в 2010–2012 гг. и получавших нимесулид в течение последнего года (не менее 3 курсов по 14 дней каждый). Средний возраст пациентов составил 42,6±10,9 года, длительность заболевания – 11,9±8,2 года, мужчин было 83,33%. Диагностику АС проводили на основании модифицированных Нью-Йоркских критериев (1984). Выполняли общеклиническое обследование (общий анализ крови, общий анализ мочи, СРБ, общий белок, альбумины, мочевина, креатинин, глюкоза, билирубин, аспартатаминотрансфераза и аланинаминотрансфераза сыворотки крови). Активность АС определяли с помощью индекса BASDAI (Bath Ankylosing Spondylitis Disease Activity Index). Оценивали подвижность осевого скелета, ротацию в шейном отделе позвоночника. Анализировали терапию, которую пациенты получали на момент госпитализации, и рекомендации лечащих врачей при выписке пациентов из стационара. Проводили анкетирование врачей (n=100) амбулаторного звена Саратова для уточнения целей и способов назначения противовоспалительных препаратов.

Результаты. Показано, что врачи амбулаторного звена применяли комплексную трехкомпонентную терапию у 53,12% пациен- тов с АС, двухкомпонентную – у 45%, монокомпонентную – у 2%. У пациентов, получавших трехкомпонентную терапию (комбинация 200 мг/сут нимесулида, 4–8 мг/сут тизанидина и местная форма НПВП), при сопоставимом уменьшении боли установлена большая подвижность в поясничном отделе позвоночника, чем у пациентов, у которых терапия включала только НПВП.
