Клиническо е течение и стандарт н а я терапия системной красной волчанки в Российской Федерации

Данные об особенностях клинического течения и лечения такого тяжелого заболевания, как системная красная волчанка (СКВ), в России практически отсутствуют.

Цель исследования ESSENCE – оценка клинической картины заболевания и его лечения у взрослых пациентов с активной СКВ в городах трех постсоветских государств (Россия, Украина и Казахстан).

Материал и методы. Настоящая публикация касается российских пациентов. Изучены данные (истории болезни) 232 пациентов, которые наблюдались в 6 центрах (Москва, Санкт-Петербург, Воронеж, Екатеринбург, Курск, Ярославль). Оценивали демографические и исходные характеристики пациентов, активность СКВ, лабораторно-инструментальные исследования, лечение и потребляемые ресурсы здравоохранения.

Результаты. Демографические характеристики пациентов с СКВ были в целом сопоставимы с таковыми в других международных исследованиях. Однако российские пациенты оказались на 4–10 лет моложе, а длительность СКВ у них была на 3–7 лет меньше, у большинства пациентов отмечалось тяжелое течение заболевания. Уровень активности СКВ у пациентов в нашем исследовании был выше. Зафиксированы частые госпитализации и незапланированные визиты к врачу в связи с обострениями. Российские пациенты, обращающиеся за повторной медицинской помощью, имеют выраженные и тяжелые осложнения СКВ. С большой долей вероятности у достаточно большого количества больных заболевание остается недиагностированным или диагноз установлен неправильно. Отмечены недостатки в лечении СКВ: противомалярийные препараты получали менее половины, а глюкокортикоиды – почти 100% больных, нестероидные противовоспалительные препараты назначали редко. Исследование ESSENCE позволяет составить представление о клинической картине СКВ в России. Эти данные могут помочь в планировании ресурсов здравоохранения в нашей стране.
